Pre-K and Kindergarten Registration

Pre-K and Kindergarten registration must be completed at the Fayette County Welcome Center (Lafayette Educational Center, 205 Lafayette Avenue, Fayetteville, GA 30214). Registration hours are 8:00 a.m. - 4:00 p.m. (closed 12:00-1:00 p.m.) For more information, call 770-460-3990 or visit www.fcboe.org.

* After registering at the Welcome Center, parents need to stop by the school office to schedule a Kindergarten screening for their child and to pick up a school supply order form.

DOCUMENTS NECESSARY FOR REGISTRATION:

Proof of Residency:
Current property tax statement (may be obtained by calling Fayette County Tax Commissioner's office at 770-461-3652) OR signed Lease Agreement OR Affidavit of Residency (obtained from Welcome Center)

Current Gas or Electric Bill:
Must display your name, address and lot number

*Students WILL NOT be placed in a class without proof of residency!

Certified Birth Certificate:
Must have notary stamp and certificate number. Certificates/confirmations of birth from a hospital are not acceptable. Copies of birth certificates can be obtained by calling the Georgia Department of Human Services Vital Records at 404-679-4701 or by visiting www.health.state.ga.us

Social Security Card:
Must be the original card. Cards can be obtained by calling 1-800-772-1213 or by visiting www.socialsecurity.gov

Photo ID of parent/guardian enrolling student

Immunization Record (Georgia Form 3231)
Obtained fron your child's physician OR the Fayette County Health Deptartment (770-461-1178, ext. 5416 - by appointment only). YELLOW Health Department forms ARE NOT ACCEPTABLE.

Hearing, Vision and Dental Certificate (Georgia Form 3300)
Obtained from your child's physician

*The Fayette County Health Department will transfer immunization records and Hearing, Vision and Dental records for you. Please contact them at the above number for further information and costs.
